Any suspicion or concern of abuse or exploitation of a child/vulnerable adult involving Caritas Georgia staff and associates must be reported as required in the Caritas Georgia Children and Vulnerable Adults Safeguarding Policy.
If you are unable to complete all of the sections of the Report Form, fill in what you do know. If there is more than one victim, please complete a separate report for each victim. The reporter's identity will not be disclosed except on a “need-to-know” basis. If the reporter believes danger is imminent to themselves or anyone involved, he/she should alert the Executive Director of Caritas Georgia or local senior management, who in his/her turn immediately informs the Executive Director.
Any suspicion or concern of abuse or exploitation of a child should be reported by CG senior management to state Care Agency by filling the “Child Protection Card” or/and call “15-05” – the hotline of the Ministry of Internally Displaced Persons from the Occupied Territories, Labor, Health, and Social Affairs of Georgia. (Besides the filling of the CG reporting form)

If an immediate threat to life exists or if emergency assistance is needed, please contact the Emergency and Operative Response Center of the Ministry of Internal Affairs – call “112” and alert local senior management immediately, who in his/her turn immediately, informs the Executive Director.
The purpose of this Report Form is to report any suspicious activity of abuse or exploitation for Caritas Georgia to assess and determine next course.
