Complaint for misconduct (fraud, corruption, bridge of codes, misbehavior, harassment, etc.)
1. Complaints for misconduct must be reported as required in the Caritas Georgia Feedback and Complaint Handling
Procedures.
2. Caritas Georgia is committed to addresing and responding to all reports.
3. Fill in here any suspicion or concern of fraud, corruption, bridge of codes, misbehavior, or harassment that happens in Caritas Georgia and/or involves a Caritas Georgia staff member or volunteer.
4. The purpose of this Report Form is to assess and determine the next course of action.
